What to expect on your first visit
During your first visit to our office, we will review your dental and medical history and take digital X-rays of your teeth and mouth. Our staff will also carry out a oral cancer screening. As part of your first visit, Dr. Williams will create your own treatment plan and answer any questions you may have.
If you have scheduled a teeth cleaning for your first visit, our wonderful hygienist will clean and polish your teeth. She will also check your teeth for cavities, early signs of tooth decay.

For your convienance, please print these new patient forms and bring them with you to our office for your appointment.
If you prefer to fill the forms out in our office, please arrive 15 minutes early for your appointment and be sure to bring your list of medications. If you have any questions while filling out the forms, please call us.
Regular checkups are important to maintain your great smile!
At Shiloh Family Dental we encourage our patients to follow the American Dental Association's recommendation of visiting your dentist every six months for regular checkups and routine cleanings to keep your mouth healthy.
